Motion To Set Aside the Default

General Order 20-3 from the Chief Administrative Law Judge motions to set aside the default can be submitted via email.

Please complete the MTSA form and chose one of the three following options:

     1.) email it to AH-MTSA@cityofchicago.org 

     2.) fax to 312.742.8222, 

     3.) file in person at 400 West Superior. 

Please select only one of the above options.  For email or fax requests, please allow at least three business days for you MTSA to be processed.  Resubmitting another request will delay the process.  Do not email and than faxed or mailed in your MTSA.

DOAH will then schedule a hearing date on which this motion will be heard and send you notification to your email. Please include a daytime phone number on this motion.
