The Chicago Department of Public Health releases Requests for Proposals (RFP) and Grant opportunities. Not-for-profit community-based organizations providing services within the City of Chicago may apply for these funds. There are times when other opportunities are available for professional services that do not require a Not for Profit status. Call or email the contact person listed on the request for proposal.

The City of Chicago, through the Chicago Department of Public Health (CDPH), is seeking to fund four community-based organizations, each in one of the priority community areas of Austin, Englewood, South Lawndale, and West Garfield Park. These organizations will serve as community conveners, delivering services and coordinating activities with local stakeholders according to the People’s Plan for Community Safety (PPCS) priorities.

This RFP supports Healthy Chicago 2025 goals of reducing violence, providing trauma-informed care, supporting collaboration among communities and the broader public, highlighting restorative justice practices, and the overall goal of creating a city with strong communities and equitable access to resources, opportunities, and environments that maximize health and well-being.

CDPH seeks a qualified delegate to complete an environmental scan of Chicago's healthcare providers' use of and access to, the national Electronic Case Reporting system, instituted by CDC. The delegate would create an evaluation and generate reports for the providers and the Department about the access, readiness and implementation needs. The vision is to highlight any significant gaps, barriers and coverage, to guide providers toward implementation using the readiness and recommendations provided by the delegate.

The City of Chicago has committed through the Healthy Chicago 2025, our community health improvement plan, to identify causes and address the racial health life expectancy gap . This RFP works directly into the themes to transform processes and improve systems of care for populations most affected by economic, racial, and social inequities. Having full participation in the eCR across the city provides the ability to identify emerging illness and act quickly to prevent its spread in all communities.
